Subject: PER – Leave Sharing Program

Purpose: To alert employees of a fellow employee in need of donated annual leave.

Expiration Date: September 30, 2007.

Angela Warden, Soil Scientist at the Tuscaloosa MLRA Soil Survey Project Office is in need of donated annual leave. Angela is caring for her father who is in the final weeks of Stage IV brain cancer.

Anyone wishing to donate annual leave to Angela should complete the AD-1043, Leave Donor Request Form, and return it to Shirley Hall-Cole in the Financial Management Section of the State Office.

Any amount of donated leave will be greatly appreciated by Angela.
