Subject:  PER – Leave Sharing Program

Purpose:   To alert employees of a fellow employee in need of donated annual leave.

Expiration Date: September 30, 2007.

Jeffrey Ethridge, Soil Conservation Technician in the Abbeville Field Office has been diagnosed with Non-Hodgkin’s Disease (Mantel Cell Lymphoma). He is currently undergoing five chemotherapy treatment cycles with a recent aggressive change to chemotherapy requiring hospitalization. His current chemotherapy treatment plan will be followed by an Autologous Stem Cell Transplantation requiring extensive hospitalization, high dose chemotherapy and a physical recovery period lasting several months. His sick leave and annual leave will expire this pay period.

Anyone wanting to donate annual leave to Jeffrey should complete the enclosed AD-1043, Leave Donor Request Form, and return it to Shirley Hall-Cole in the Financial Management Section.
